Our Mental Health Team are looking to recruit a full time Mental Health Customer Coordinator to join their busy team based in Trowbridge and covering West Wiltshire. LIVE WORK SUPPORT Our Mental Health Team work across all age groups for the statutory mental health work which includes children/adolescents, adults (including those with a learning disability), and older people. The function of the team is to provide social care interventions to those people meeting the threshold for secondary mental health care. This includes assessment, care management, social work interventions, multi-disciplinary working, risk assessment and management. A particular challenge of this role is to ensure customer’s experience of the service as a whole (Health and Social Care) is as integrated and seamless as possible. About the role As a Customer Coordinator you will act as the key point of contact for customers, including those who fund their own care, who require our services and coordinate any care, advice and services that the customer may require. You will support the assessment of adults with mental health problems who have eligibility under the Care Act and other legislation, including assessing risk and planning, implementing and reviewing services as appropriate to enable independent functioning and ensure that service users and their carers are protected from harm, referring onto other services where appropriate. The role will require you to carry out assessments of customers or complete reviews of customers in placements or in receipt of packages of support from the Council whilst managing your caseload, prioritising work and tasks appropriately and using effective time management skills. You will work within guidelines and using own discretion, identify when specialist involvement is required, make the appropriate referrals and ensure effective liaison with the other professionals. About You The work is fast-paced, and the right candidate will have the ability to work collaboratively with multi professional teams. You will hold an NVQ level 4 in Health and Social Care or equivalent or the ability to work towards this qualification. You should have experience of working in a health or social care setting and have excellent communication skills. You will have relevant or transferable previous experience in a Mental Health or Adult Care setting advising and guiding clients. You will have a good awareness and understanding of Government initiatives in relation social care and mental health and be an active team player by contributing to the development of the service, ensuring the customer is always at the centre of such developments. You will be an active team player and an excellent communicator Interview date will be confirmed upon shortlisting.
