Mersey Care NHS Foundation Trust and Liverpool University Hospitals NHS Foundation Trust are working in partnership to develop a brand new service for older adults to improve patient outcomes, reduce lengths of stay and bring together the expertise in to an integrated physical and mental health offer.
The service is based on a Nursing and Therapy rehabilitation model and will include a step up and step down offer at a sub-acute level. Our workforce will consist of expertise from our Community Intermediate Care Bed Base, Community Therapies, Integrated Care Teams, Community Nursing, ICRAS, Psychological Therapies and Mental Health expertise.
We are looking for additional staff to bolster this expertise and expand the workforce to cover the additional clinical model and the extra beds built into the model.
The service will be based within the Aintree Hospital site grounds and the estate is currently being designed and renovated to meet the clinical model and the needs of this patient group. The service will have 69 beds in total organised into pathways dependant upon need.
This is an exciting time to become part of a fully integrated service aimed at recovery of the whole person. Our combined expertise, revised pathways and community connection will enable more timely discharge in a holistic way.
