We focus on delivering care as close to home as possible. The Crisis Response and Home Treatment Team is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services by 2021. Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible by providing a variety of highly intensive interventions at home. We require clinicians who can bring their own professional expertise to a fast-paced team, and who are confident in managing risk in the least restrictive way. By its nature, crisis work requires professionals who have a broad range of skills including being skilled mental health assessors with a proven ability to signpost service users towards the most effective treatment pathways. Crisis can manifest with a range of needs and whether you have a background in mental health nursing, social care or occupational therapy we want you to use your skills as part of a team to develop individualised care packages for our service users and their families. Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police and our street triage teams. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including: Psychiatric Liaison Overnight Response Team Assessment Teams Street Triage and Health Based Places of Safety The successful applicants will participate in a duty rota of early, late or long day shifts covering 7 days a week and we are committed to supporting staff to balance home and work life. We are a friendly and supportive team with strong leadership and direction. Our work is interesting and varied with plenty of opportunities to learn new skills both in the workplace and via our excellent Learning & Development team. The role is predominantly based in Aylesbury and may require some wider travel within the county. We would welcome clinicians who are keen to develop or further enhance their skills in a variety of areas and to help shape this exciting and developing service.

New starters have a 6 month probationary period. This, together with the induction process aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale. Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments.
